    The Stage of Change Scale is a simple rating form that consists of 7 items assessing early childhood caregivers’ readiness to change their child care practices. The scale is based on the Transtheoretical Model of Change, a framework developed in the field of behavioral health and endorsed by the National Institutes of Health.

    • Awareness of effects of behavior on children
    • Confidence in overcoming obstacles to change

    Administration: Mentors/coaches working with center – or home-based early childhood caregivers/teachers (one form per caregiver/teacher)

    Scannable paper forms can be returned to Children’s Institute to quickly score and summarize the data you’ve collected. Profiles, percentiles, and reports showing total strengths, needs, and trends are provided where applicable. Pre/post evaluations and data files are also available.
