    The Professional Summary Report (PSR) summarizes a child’s program experience to produce a cumulative list of information regarding the child. The supervising mental health professional completes the PSR based on observation of the child and input from service providers and teachers. Typically, the PSR is used for the Primary Project program.

    Section one describes changes in children's school behaviors-strengths, weaknesses, and performance-since the time of referral. Section two describes why the child is leaving the program, and gives the professional's recommendation for future program involvement.

    Scannable paper forms can be returned to Children’s Institute to quickly score and summarize the data you’ve collected. Profiles, percentiles, and reports showing total strengths, needs, and trends are provided where applicable. Pre/post evaluations and data files are also available.
